1. Click on the "Join Co-op" tab
2. Type in your sponsor's ID number and click on the blue "Search for Sponsor ID" button. (Font Size 12pt)
3. Verify that it is the correct name of your sponsor.
4. Click on the blue "SELECT SPONSOR" button on the left side.
5. Another screen will come up to fill in your personal information. Once completed, click on the blue "SAVE" button at the bottom.
6. You will go to another page of instructions - click on the blue 'ONLINE PRINTABLE APPLICATION"
7. A screen will come up with your information inserted into the application form. Your new ID number is printed at the top -- you may begin using it immediately. The computer creates your ID number. Old format numbers will continue to work.
8. At the bottom is a blue "PRINT CO-OP APPLICATION" button. Print out completed form. (Please print 2nd copy for your records)
9. Sign and date the application.
10. Enclose completed application, along with $20, and mail it to the address at the bottom of the form.
11. You may use regular, priority, or express mail. You may use delivery confirmation, but DO NOT require a signature or it will delay your processing.
Make Sure You enter your sponsors ID!
